About Healthy Hearts
African Americans and Latinos in San Francisco are at greater risk for heart disease and diabetes than the general population. Opportunities for physical activities that are accessible, culturally relevant and convenient can increase the likelihood of participation and help create long-lasting behavior change.
The Healthy Hearts SF Campaign, with funding from the Centers for Disease Control (CDC), aims to increase cardiovascular health in both the Latino and African American populations living in the census tracts identified by the CDC as having the highest racial and ethnic health gaps in the city. Among these census tracts, Healthy Hearts is focused on communities where 30% or more of the population is living at 200% below the Federal Poverty Level, and 25% do not have a high school education.
One of the primary goals of the program is to raise awareness among community members, community agencies, and clinical teams about the free fitness resources available to them. Residents of the target census tracts can dial 211 for a list of free, safe, and accessible physical activity opportunities offered around the city.
Healthy Hearts SF is also implementing a “free exercise prescription” initiative through participating clinics. These providers will refer African American and Latino patients to free physical activity opportunities such as Zumba, group walks, or water aerobics provided by Healthy Hearts SF partners.
